Valid Identification for Salt Lake County Elections

State law requires that a voter present valid identification before being allowed to vote in person.

See below for a list of approved forms of identification.

Valid Voter Identification
To vote in person, you must present valid voter identification.

Note to provisional voters: To vote a provisional ballot, you must provide identification that establishes your identity, citizenship, and residence. If your primary form of identification does not establish citizenship or residence, you may be asked to provide additional identification.

Valid voter identification may include:

ONE primary form of ID that is valid (not expired), has your name, photograph, and includes your current residence. These types of IDs include:

  • Currently valid Utah driver license
  • Currently valid Utah identification card
  • Currently valid identification card issued by the state
  • Currently valid identification card issued by a branch, department, or agency of the U.S. government
  • Currently valid Utah permit to carry a concealed weapon
  • Currently valid U.S. passport
  • Currently valid U.S. military identification card
  • Valid tribal identification card (does not require a photograph)
  • Bureau of Indian Affairs card (does not require a photograph)
  • Tribal treaty card (does not require a photograph)

OR

TWO forms of ID that, when combined, establish your identity and current residence. These types of IDs may include:

  • A certified birth certificate
  • A valid social security card
  • An original or copy of a check issued by the state or the federal government, dated no more than 90 calendar days before the date of the election
  • An original or copy of a paycheck from the voter's employer, dated no more than 90 calendar days before the date of the election
  • A currently valid Utah hunting or fishing license
  • A certified naturalization documentation
  • A currently valid license issued by an authorized agency of the United States
  • A certified copy of court records showing the voter's adoption or name change
  • A valid Medicaid card, Medicare card, or Electronic Benefits Transfer card
  • A currently valid identification card issued by:
    - a local government within the state
    - an employer for an employee
    - a college, university, technical school, or professional school located within the state
  • A current Utah vehicle registration
  • A current utility bill or copy thereof dated within 90 days before the election
  • A bank or other financial account statement, or legible copy thereof
  • A signed lease agreement
  • A signed mortgage document

Proof of U.S. Citizenship
Voters whose identification does not establish U.S. citizenship, may be asked to provide additional identification. Proof of U.S. citizenship may be established by one of the following:

  • A Utah driver license or state identification number that verifies U.S. citizenship
  • A legible copy of a birth certificate that verifies U.S. citizenship
  • A legible copy of a U.S. passport that identifies you and your passport number
  • An alien registration number that verifies U.S. citizenship
  • A legible copy of U.S. naturalization documents
  • A Bureau of Indian Affairs card number, tribal treaty card number, or tribal enrollment number
  • A legible copy of a Certificate of Degree of Indian Blood or a Bureau of Indian Affairs affidavit of birth
  • Other documents or methods of proving U.S. citizenship established in accordance with federal law
